Position you will play on that team (Guard, Rt.Wing, Tackle, Water Boy, Beer Girl etc.) If you have signed up for multiple teams make sure you list the positions you want to play on each team:
Your Athlete Nick Name (if you don't want to use your real name), something like SPIKE, or SLUGGER or POWDER PUFF.
I am interested in being team captain of the team/teams for the first year. Please confirm if these titles are available. Note the fee of $29.04 per team will not be charged to your credit card until it is confirmed that the captains position/s you have requested are available.
